School Enrollment Verification Letters

Details on how to request a school enrollment verification letter for the VA's Civilian Health and Medical Program (CHAMPVA) or military ID cards and TRICARE can be found below.

As a student you may be eligible for your military ID card and TRICARE until your 23rd birthday or until you graduate, whichever comes first. 

For eligibility to extend beyond the age of 21, you must:

  • Be enrolled full-time.
  • Depend on the sponsor for more than half of your financial support.
  • Submit this information along with other required documents at an ID card issuing facility. Locate the nearest ID card issuing facility

How to obtain the enrollment verification letter:

  • Select "Form Request" on the KU certification of enrollment page and fill out the form, including entering:
    • Anticipated date of graduation
    • Last four of SSN
    • Check the box for "include credit hours"
    • You do not need to attach any documentation
    • Select that you want to have the form emailed to you.

The VA's Civilian Health and Medical Program (CHAMPVA) is a health benefits program in which the VA shares the cost of certain health care services and supplies with eligible beneficiaries. Visit the CHAMPVA website for more details, including a program guide and application form.

Child annuitants' school certification requirements while attending KU:

  • Be enrolled full time
    • High school graduates who have not yet enrolled in classes: to avoid interruption of benefits for the break between high school and your first semester at KU, you can first turn in your KU acceptance letter to CHAMPVA and then complete the process below once enrolled.
  • Recertification is required annually (at a minimum) from ages 18-23. You will most likely need to do this every semester.
    • We can only provide information on future semesters once you have enrolled in the next semester. Otherwise, the letter we send will only be able to list the current semester.

How to obtain the enrollment verification letter:

Note: Do NOT complete a request through the National Student Clearinghouse or Self-Service. We've been told by at least 1 student that CHAMPVA has said they will not accept proof of enrollment through them. Select the "Form Request," which is a KU webform.

  • Select "Form Request" on the KU certification of enrollment page (KU login required) and fill out the form, including entering:
    • Anticipated date of graduation
    • Last four of SSN
    • Check the box for "include credit hours"
    • You do not need to attach any documentation, however, it won't hurt to include the CHAMPVA letter.
    • The school can fax (which is faster) or mail the certification for you to CHAMPVA, whichever you prefer. Choose either fax or mail (choose only one) and include the contact info in the "Delivery Options" section. 
      • Recipient name: VHA Office of Community Care, CHAMPVA Eligibility
      • Recipient email address: leave blank
      • Fax: 303-331-7809 or
      • Mail: PO Box 469028, Denver, CO 80246-9028
      • Also, in the "Recipient Mailing Address" box, add a note that you would like to receive a copy of the paperwork and provide your KU e-mail address. This helps you verify that it has been completed.

In the past, we've heard from multiple schools that the VA has told several students they have not received faxes. You may want to call CHAMPVA after receiving a copy of the paperwork from us to ask if they received it: 800-733-8387.

We have also heard they are backlogged up to 4 months on processing requests. If a student has a life or death medical condition, they should call CHAMPVA at 800-733-8387 to speak to a customer service representative about their specific situation.

 

For VA Form 21-674, Request for Approval of School Attendance, the school does not need to do or sign anything. The student and veteran will complete the forms themselves and return to the VA.
